Coordination strategy for stakeholders' engagementThis Deliverable will detail GerOnTe strategy for all stakeholders involvement related to the creation of HolisTM GV This document will include the specific goals of the partners strategy and a timeline for all cocreation sessions ie Focus Groups and smallscale pilots in France Belgium and the Netherlands The D61 will also include an action plan for all Focus Groups including the needs in terms of required time logistics specific goals of each organised Focus Group human resources and general methodology to moderate the Focus Group to engage discussion Each local partner will contribute to the deliverable with countryspecific consideration such as ethical committees to be considered The recruitment process ie recruitment channels participant numbers for the cocreation session will also be detailed in this deliverable
Practical guidebook and tools for Focus Groups in WP1-5This deliverable will detail the common methods and tools for organizing the co-creation sessions about Holis(TM) creation in the framework of the technical WPs 1 to 5. This deliverable will be a guidebook to be addressed to the host of the Focus Group explaining moderation of the discussion, collection of the participants’ feedback and how to report the users’ feedback for the consortium for further implementation in the GerOnTe system. This document will also include the necessary support tools for the participants (information documents, informed consent, presentations). It will specify the following aspects for the Focus Groups: project presentation, Focus Group objective presentation, guideline for leading Focus Group discussions as well as recommendations for collection users’ feedback during the Focus Groups.The guidebook will be updated at M9, M24 and M48
Care pathways in the 3 trial countriesThis deliverable is intended to describe the current care pathways for older multimorbid patients within the three pilot countries in terms of professionals involved, care settings and activities. The description of the care pathways in the 3 trials countries during GerOnTe implementation will be updated at M58.
Literature review on the cost-effectiveness of healthcare services for older multimorbid patientsThis report will describe what are the existing methods and models suitable to capture the main potential of GerOnTe and will identify the distinctive feature of service and delivery interventions that pose challenges to the economic analysis of GerOnTe model
Dataset of self-management recommendations for patient-driven improvement of independent livingThis deliverable aims to describe 15 common symptoms and related selfmanagement recommendations that patients can access during their treatment trajectory

An original design will be created with the help of a professional designer for the visual identity and graphical charter of the project The graphical charter will be used for all project communication activities
Dedicated Website, Twitter and LinkedIn accounts liveLinedIn and Twitter accounts dedicated to the project will be opened and managed continously for wide communication of project and other related information A dedicated project website will be created and live at M3 regularly fed with updates throughout the life of the project
BrochureDuring the early months of GerOnTe for promotion and communication purposes a professionally designed brochure will be produced It will serve as an advertisement tool electronic version for download on the project website and a printed version describing the main objectives of the project summarized with an overview of the consortium partners as well as a brief outline and description GerOnTes commitment and aims It will essentially provide a quick on the spot core information about GerOnTe
This deliverable aims to list 1 the five most common patient profiles with for each of them at least 13 health professionals listed 5 HPC core members and 8 additional professionals depending on the comorbidities geriatric needs and hospital resources 2 at least five medical data components that need to be available in the common medical file for all patients and 3 a core dataset of at least 4 frailty components 3 intrinsic capacity components and 3 patient preferences that need to be included in each CGA
This deviverable aims to present a protocol for CGA adapted from geriatric medicine to incorporate factors identified in D1.3 that are relevant for intrinsic capacity and self-management.
