This will identify the key stakeholders to engage with during the requirements gathering phase, care pathways of the patient and challenges in supporting patients at home.

Ecosystem Trial Strategic Plan

This document will outline the framework for trials: where, how, with which tools, which roles and partners, third party legal and ethical requirements. Plans will include: Protocol for conducting “friendly” pilot study (M9); Protocol for conducting large-scale trial (M23); Protocol for conducting the feasibility study (M28). This deliverable will link to milestone 1. Due date is month 9 with updates in months 23 and 39.

Development of a set of person-centred analytical methods for risks and outcomes. The initial report will be delivered in month 10 with updates in months 22 and 39.

The survey will look into the multiple factors that determine the uptake of integrated care platforms or the resistance against it among the different stakeholders (health professionals, social care workers, providers, funding agencies).
